Shape and background first
Federal designation R10-11a in the MUTCD, the national standard every state signs to. Mounted on the signal pole or mast arm facing the lane it restricts, usually at intersections near schools, hospitals or heavy pedestrian crossings.

Standard: Use the R10-11a sign when the approach has both circular red and red arrow indications.

How states handle it
A handful of cities, including New York City, ban turns on red citywide unless a sign specifically says otherwise, which flips the usual default.
What the test wants you to know
The mistake people make
This sign only overrides the default right-on-red rule for the specific approach it is posted on, not the whole intersection. Some versions carry a small time plaque underneath, so the ban might only run during school hours or the evening rush rather than all day.
