Why half of this list is a set of questions
There is a version of this page all over the internet that lists the same documents for every state and sounds completely authoritative. We will not write that one. Document requirements are set jurisdiction by jurisdiction, they change, and the difference between two documents proving residency and one is exactly the difference between driving home with a permit and driving home without one.
So the checklist is split. The top half is built only from figures we read on a named agency page or in state law, with the page linked and the date we read it beside every line. Across the 51 jurisdictions here, 24 currently have at least one such figure attached to a visit. The bottom half is deliberately written as prompts rather than answers: the categories every agency asks about, each pointing you at the page that says what your state accepts.
The three visits are not the same trip
A permit visit is about proving who you are, where you live and how old you are, plus the knowledge test and the fee. A road test visit is about the car and your record: the permit still in date, the holding period satisfied, the practice log signed, and a vehicle that passes inspection before the examiner will get in it. A licence visit is usually the shortest of the three, and the one people most often turn up to with the wrong proof of address because they did not think it needed anything.

Two things worth doing the day before
Check whether your office takes walk-ins or appointments only, and check which payment methods it accepts. Those two are the most common reasons a trip is wasted that have nothing to do with documents at all, and both take two minutes to confirm on the agency's own page.
Then plan to bring originals. Agencies commonly refuse a photograph of a document on a phone, and a certified copy is not the same thing as a photocopy, so check which form of each document yours will take.
